I understand that all uniforms and accessories are company property and they must be returned upon termination.
I understand that the wearing of the prescribed uniform is a condition of employment with this Company.
I acknowledge receipt of the uniform(s) as specified above, which has been provided to me.
I will return the uniform(s) to my Supervisors immediately upon termination of my employment.
In the event that I fail to return the uniform(s), I hereby authorize the company to deduct the replacement
value of the uniform(s) from any of my paychecks.
I understand and acknowledge that the company has the right and will take whatever actions necessary to recover
the reasonable value of the uniform(s).
Should such actions be necessary, I understand and agree that I will be responsible for all reasonable costs incurred
in the collection of the amount due including but not limited to attorney's fees, interest and costs.
I understand that my uniform(s) must be clean at all times and may be cleaned by the uniform service if I turn
in on a weekly basis. The uniforms may only be worn while working and traveling directly to and from work.
