Shirts ____________ $25.50 each\n3. T- Shirts ____________ $15.50 each\n \n __________________________________________________________________<\/strong><\/em><\/strong><\/em>\n I understand that all uniforms and accessories are company property and they must be returned upon termination.\n\n I understand that the wearing of the prescribed uniform is a condition of employment with this Company. \n I acknowledge receipt of the uniform(s) as specified above, which has been provided to me. \n I will return the uniform(s) to my Supervisors immediately upon termination of my employment. \n In the event that I fail to return the uniform(s), I hereby authorize the company to deduct the replacement \nvalue of the uniform(s) from any of my paychecks. \n I understand and acknowledge that the company has the right and will take whatever actions necessary to recover \nthe reasonable value of the uniform(s). \nShould such actions be necessary, I understand and agree that I will be responsible for all reasonable costs incurred \nin the collection of the amount due including but not limited to attorney's fees, interest and costs.\n I understand that my uniform(s) must be clean at all times and may be cleaned by the uniform service if I turn \nin on a weekly basis.
